Is the niece killing the good old doctor? now you can stop it:
When doctor gets lonely, proceed as usual presenting him the Ranek "niece", when he asks for information regard her, ask her aunt about her to find out shes a ruse.
At this point go grab your red phone and ask info on the niece by telling them shes a "fake", they will inform you shes an imposter with previous poisoning convictions.
Now you can go the this "niece" and extort her out of 5K, basically make the deal.

As soon as you do this deal, go make a report on the b*itch, JAIL time for her, 5k for you. Doctor will even give you tickets for 5k more (sell to trader)

By now you can pay up your kid´s health bill

at the right time, speak with her aunt. then ask about her.
